Accident - Companion
The Accident Companion supplemental plan is designed to be paired with health insurance to assist with the payment of the deductible and coinsurance in the event of an accident. Choose from four plan options with lump-sum benefits ranging from $2,500 to $10,000. Form CH-26122-IP (01/11)
Accident - Critical
Chesapeake’s Critical Accident Direct plan pays a lump sum benefit for an accidental injury resulting in quadriplegia, paraplegia, hemiplegia, third degree burns to 10% of the body, second degree burns to 20% of the body, coma, the loss of sight in both eyes, or hearing in both ears. Benefits are per insured person, per lifetime: $10,000, $15,000, $20,000, $30,000, $40,000, $50,000 or $60,000. Form CH-26123-IP (04-11)
Accident - Lump Sum
The Accident Direct plan pays a lump sum benefit for accidental injuries resulting in a hospital confinement and based on the number of days confined. Benefits renew annually. Benefits: $5,000, $15,000, $20,000 or $25,000 per insured person. Form CH-26118-IP (01/10) The ProtectFit Plus plan offers supplemental insurance for accident-related care through a blend of lump sum benefits and daily benefits in both a High or Low plan option. Benefits under these options include hospital confinement, outpatient emergency treatment, major diagnostic exams and follow-up treatment for accidental injuries. There are benefits for accidental death and dismemberment, emergency ambulance, and the High Plan option includes a monthly disability benefit for covered adults. Form CH-26110-IP (06/09)
Cancer
The CancerWise plan pays a one-time benefit on first diagnosis of a malignant internal tumor or malignant melanoma. Benefits: $20,000, $30,000, $40,000 or $50,000 per insured person, per lifetime. Form CH-26055-IP (05/07)
Critical Illness
Chesapeake’s Critical Illness Direct plan pays a lump sum benefit on a first diagnosis of a list of covered illnesses, including: 100% for Advanced Alzheimer’s, ALS, illness-induced Coma, Stroke, Life-threatening Cancer, Heart attack, End-stage renal failure, and Major organ transplant. 25% for Coronary artery bypass, benign brain tumor, cancer in situ. Dental
Chesapeake’s Basic and Premiere Dental PPO plans can complement most health insurance plans by providing a range of covered services and savings through Careington’s Maximum Care network of 135,000 access points. Hospital Indemnity
The Hospital Confinement Direct plan pays a daily benefit for hospital confinement due to an illness or injury, subject to a waiting period for illness. Benefits: $500 per day, 365 days maximum. Form CH-26116-IP (01/10)
Short Term Disability
Chesapeake has plans that pay a monthly benefit for a diagnosis of total disability up to 12 or 24 months, subject to an elimination period and income limitations. Accident Disability Direct provides coverage for accidental injuries only and the Income Protection Direct plan addresses both illnesses and accidental injuries. Vision
Chesapeake’s Premiere Vision plan provides a routine eye exam for each insured person, per 12 month period, with no co-pay. There is a $10 co-pay for frames, uncoated plastic lenses, or corrective contacts up to a $120 allowance.
